Opt-in/Opt-out Question

If you need to have an Opt In/Opt Out question you can create one using the Waiver/Terms & Conditions question type.

To add an Opt In/Opt Out question do the following:

  1. Click "Edit Fields & Questions" in the "Setup" timeline
  2. Click the "Waiver / Terms & Conditions" question
  3. Update the Reference Name if needed
  4. Edit the Text/HTML using your Opt In/Opt Out language
  5. Click "Submit and Return to Questionnaire"
  6. Click "Edit Answers" and add/update the defined answers
  7. Click "I am done! >> Return to Questions"
